Please fill in the information on the form at the bottom of the page to register for a make up appointment.  Please read the guidelines below.

* Deadlines and make up opportunities are listed for each league below.
* Make ups will only be available on these dates at these times listed for your league,
* No make ups will be accepted after the last date listed for your league.

IF NO ONE REGISTERS BY 1:30PM ON THE DAY OF THE STUDIO MAKE UP SESSION, THE SESSION IS CANCELLED.
